The Medication Management Review initiative (MMR) is funded by the Department of Health and Ageing and auspiced by the Pharmacy Guild of Australia.

MGPN employs an accredited Pharmacist, Len Wade to deliver this program.  The primary objectives of the initiative are:

• Support and facilitate communication between GPs and pharmacists regarding medication management reviews;
• Assist GPs, pharmacists and other health professionals to identify patients who may benefit from a MMR, including upon discharge from hospital; and
• Link into other local quality initiatives such as Medication Advisory Committees (MACs). 

The program is delivered by individual face to face visits and small group education.
